In person at reception or by phone on +48 697 120 100, during opening hours. A representative can also register you. Reception will tell you about the services available, waiting times and the documents you need.

An ID; proof of insurance if the eWUŚ system does not confirm your entitlement (you can also sign a declaration available at reception); proof of any additional entitlements; and your referral – the original must reach us within 14 days of booking.

Per referral – up to 10 treatment days, at most 5 treatments a day. A physiotherapy referral has to be registered with us within 30 days of being issued.

All test results and imaging related to your condition and any hospital discharge summaries. At the first visit you fill in a form naming who may receive information about your health and records.

Yes – you can choose the specialist and the date from those available.

Come in sportswear with a change of shoes, without jewellery or a watch. Valuables are best left at home. Check in at reception before your first session.

Let us know as soon as possible, at least 3 hours before the visit or session. A missed appointment without notice removes you from the waiting list unless it was beyond your control. If we have to move a date (e.g. the doctor is ill), we will call you.

Tell reception on the day of the visit or first session. Invoices for paid services are issued monthly; the price list is available at reception.
