If a patient with a severe disability cannot get to the centre on their own, a physiotherapist comes to their home – covered by the NFZ.
How it works
- the family doctor decides – the referral lists the indications and other conditions
- up to 80 treatment days a year; unused days carry over to the next year
- the referral must reach us within 30 days of being issued
What to bring
- a referral to the physiotherapy department (1310) or the home rehabilitation team (2146)
- the original certificate of severe disability and a copy
- a phone number – we will call to arrange the dates
We also help with NFZ and PFRON applications for orthopaedic supplies and medical devices – ask at the medical shop.
